Vendor Application Form

NOTE:

1. All vendors selling goods and services MUST know their New York State Tax Certificate of Authority number when filling out the application below. If you do not have a New York State Tax Certificate, here’s how to obtain one.

All exhibitors MUST display their New York State Tax Certificate of Authority number the day of the event.

2. All food vendors MUST obtain a permit from the Suffolk County Department of Health AND insurance.

**PLEASE NOTE: If you are a food vendor/truck, it is your responsibility to ensure that you are compliant with all County, Town, etc., requirements.**

RULES:

1. All food Vendors MUST provide a certificate of insurance listing Farmingville Hills Chamber of Commerce, Inc., PO Box 30 Farmingville, NY 11738 as additionally insured. Coverage requirements: 1 million per occurrence, 2 million aggregate per policy. Certificates are to be emailed to membership@farmingvillechamber.com.

2. All food vendors must be properly licensed, and obtain a permit from the Suffolk County Department of Health.  **PLEASE NOTE: If you are a food vendor/truck, it is your responsibility to ensure that you are compliant with all County, Town, etc., requirements.**

3. All exhibitors selling goods and services MUST display their New York State Tax Certificate of Authority number the day of the event.

4. A vendor cannot sell the same merchandise as the store owners behind them. If this situation should arise, do not set up, and please bring it to our attention immediately, and your space will be moved. The Chamber of Commerce reserves the right to move vendors at any time during the course of the day.    

5. Please be sensitive to the fact that people live on the avenue/street and, please refrain from making unnecessary noise, particularly in the morning. For example, please do not throw your canopy pipes on the street; please gently place them down. Please refrain from using your horn.  PA systems and loud music are not permitted in individual vendor spaces.

6. Use of sidewalk space is prohibited. Vendors must place their tent back legs against the back of their spot, keeping the walkway consistent for attendees.

7. THIS FAIR IS RAIN OR SHINE.

8. Vendor set up is no earlier than 7:30 am – all vehicles must be off the road by 9:00 am.

9.  NO VEHICLES will be permitted to enter the fair space before 5:30 pm. Violators will be banned from future street fairs. Vendors must stay for the duration of the Fair.

10. You may bring a generator, but it must be grounded.

NO REFUNDS – NO CANCELLATIONS – NO TRANSFERS – NO CREDIT

** Members must attend 5 General Membership meetings, for a total minimum of 5, since January 2026, to receive discounted Chamber Member pricing. **
